Question: How do I create/add a custom form?
Answer:
Integrity Data will be happy to assist you with creating any Custom Forms you may need. Unfortunately, this is not a process customers can complete on their own. Due to the extra work involved, there is a fee for this service and will require a change order. If you need a Custom Form, please let your Onboarding Specialist know.
Question: How do I assign a custom form or PDF Signature Page?
Answer:
Review the knowledgebase article in the Integrity Data Resource Center linked here: Signature Pages or Custom Forms Assignments.
You may also access knowledge base article links from within the HR software. From your HR Dashboard, click the HR Admin tab and scroll to the Support window. Click on Knowledge Base and locate the article titled Signature Pages & Custom Forms.

Question: If I assign a custom form or PDF signature page, where will it be stored when completed by my employee?
Answer:
You can find the signed documents by going to your HR Dashboard and clicking the HR Admin tab. Scroll to the Employee Maintenance menu and select Documents (Employee).
