Phone Number: Enter your phone number in the following format: XXX-XXX-XXXX.
Approximate Number of Full-Time Employees: Enter the approximate number of employees that
you reasonably expect to work in excess of 30 hours/week.
Approximate Number of Part-Time Employees: Enter the approximate number of employees that
you do not reasonably expect to work in excess of 30 hours/week.
